Q&A with Dr. Jeffrey Epstein

Impact Wealth: You’ve devoted more than 30 years to advancing hair restoration, and your name has become synonymous with natural results. What is it that continues to inspire and challenge you in this work after so many years?

Dr. Epstein: Watching the field evolve and helping drive that evolution. Thirty years ago, transplants looked like plugs; today, it’s refined aesthetics, smaller instruments, tighter placement, and better planning. My greatest satisfaction is refining techniques so results remain undetectable and patients benefit from the best version of every method.

IW: There are many doctors offering hair restoration today, but few who reach the level of consistency and artistry your patients describe. In your view, what truly separates a good surgeon from a great one in this field?

Dr. Epstein: It starts with focus and intent. A good surgeon can achieve a passable result. A great surgeon obsesses over every single graft: direction, angle, and density. They perform all the aesthetic steps themselves and work with a dedicated, full-time team that shares their precision. They don’t delegate artistry. Every incision is deliberate. In my practice, we treat every case as if our reputation depends on that single result because in many ways, it does.

IW: The term “authority” gets used often in aesthetics, sometimes loosely. From your perspective, what defines true authority in hair restoration? Is it experience, education, innovation, or something deeper?

Dr. Epstein: True authority is cumulative…it’s earned, not claimed. It’s the result of tens of thousands of procedures, years of refinement, constant learning, and the respect of your peers. It’s when other surgeons, including leaders in plastic and facial plastic surgery, refer their own patients or family members to you. Authority isn’t built through marketing or social media; it’s built through consistent excellence and trust. When patients meet me, they can feel that I’ve dedicated my life to this one discipline.

IW: If you were the patient rather than the surgeon, what would you look for when choosing a doctor for something as personal and permanent as a hair transplant?

Dr. Epstein: I would look for specialization above all else. You want someone who has made hair restoration their life’s work, not one of several menu items. Ideally, a surgeon in their 50s or 60s who has mastered both the technical and aesthetic sides of the craft. Board certification in facial or plastic surgery is important, but more important is that they personally design and execute the work. They should have hundreds of before-and-after results you can review across ages, ethnicities, and genders. And they should make you feel that your result matters as much to them as it does to you.

IW: You practiced for years in New York before focusing on Miami. Today, many people still travel abroad for cheaper or faster procedures. Why do you believe Miami has become the destination for those seeking the highest standard of care?

Dr. Epstein: Miami has evolved dramatically; it’s no longer the “second-tier” option it once was. The city has become a true center for medicine, aesthetics, and innovation. What makes Miami ideal for hair restoration is the accessibility: patients can fly in, have surgery the next day, and fly home shortly afterward. With our no-shave FUE approach, most are fully presentable within 24 hours. The bigger picture, though, is that you no longer need to fly to Turkey or Colombia for excellence. You can have it done here, safely, by a surgeon who’s dedicated their life to the craft.

IW: You’ve gained a reputation as the surgeon who “fixes everyone else’s work,” which suggests both immense skill and a lot of responsibility. What are the most common issues you see, and how do you approach these reparative cases?

Dr. Epstein: Many of these cases involve limited donor supply, scarring, or unnatural design. Often, the patient has been treated by clinics that rely on technicians instead of surgeons. To correct these issues, I’ve developed techniques that let us reuse viable grafts, carefully harvest from other areas—including the beard when appropriate—and rebuild density in a way that looks natural again. But beyond the technical challenge, there’s an emotional one: rebuilding trust. Many of these patients come to me skeptical or disappointed, and the most rewarding part is seeing their confidence return.

IW: There’s increasing discussion about robotics and automated systems in hair restoration. You’ve remained entirely manual in your approach. Why?

Dr. Epstein: Because hair restoration isn’t a mechanical problem, it’s an artistic one. Robotics can extract follicles, but they can’t design a hairline that harmonizes with someone’s bone structure or ethnicity. Machines are consistent, but they lack judgment. Every angle, every density transition, every curve of the hairline should be sculpted, not stamped. That’s why we still perform every aesthetic step by hand.

IW: There’s also a lot of public interest in stem cells and regenerative therapies. What’s your take: revolutionary breakthrough or premature promise?

Dr. Epstein: It’s promising but not proven. I’ve participated in research and remain optimistic, but at this stage, too many of these treatments are being marketed without clear scientific validation. Some may even be unsafe or unethical. I encourage patients to stay grounded: hair restoration surgery remains the gold standard for permanent, natural results. The future may bring new tools, but expertise will always matter more than technology.

IW: You’ve mentored more than a hundred young surgeons, trained fellows, and now lead your own network under the Foundation Aesthetic Hair Restoration Method. How important is mentorship to you personally?

Dr. Epstein: It’s a responsibility and a privilege. Just as I was mentored by great surgeons early in my career, I believe in passing on both technical and ethical standards. I’ve trained surgeons from around the world, and through the Foundation, I’m ensuring that the next generation maintains this level of quality. Teaching keeps me sharp, but more importantly, it helps preserve the integrity of the specialty.

IW: What has been your proudest professional recognition to date?

Dr. Epstein: Receiving the Lifetime Achievement Award at The Aesthetic Show was meaningful because it came from peers who understand the work that goes into true mastery. But in truth, the recognition that matters most comes from other surgeons who send me their VIPs and from patients who look in the mirror and see themselves again.

IW: You’ve built a career known for precision and ethics. How do you personally balance the business side of aesthetic medicine with maintaining that level of care?

Dr. Epstein: By remembering the Hippocratic oath: do no harm. It’s easy to lose sight of that when financial incentives enter the equation. I’ve reached a point in my career where my motivation isn’t profit, it’s perfection. I limit the number of surgeries I perform so I can give each patient my full attention. It means fewer procedures, and sometimes waiting lists, but it also means every patient gets my best work. Quality over quantity is non-negotiable.

IW: Outside the operating room, what keeps you grounded, balanced, and inspired?

Dr. Epstein: Balance itself. My family, my wife—who’s also a plastic surgeon—our children, our home. Travel, exercise, time with my dogs, being on the water. Skiing in Colorado is another way I find my peace.  These things keep me connected to what matters most. Surgery fulfills a part of me, but life is much bigger than that. Maintaining that perspective makes me a better doctor.

IW: Looking back at your entire journey, how do you define success today?

Dr. Epstein: Success, to me, is balance and integrity. It’s doing the best work possible, helping others, mentoring, and knowing I’ve upheld the trust that so many people, patients and colleagues alike, have placed in me. At this stage, it’s less about achievement and more about legacy: leaving behind a field that’s stronger, more ethical, and more beautiful than when I entered it.
